Cardiocom is the Gold Standard for CHF DM programs
Cardiocom's hospital based clients range from a 47-site national hospital chain with $4.1 billion in revenues and 10,000 physicians to smaller single sites with less than 350 beds. Cardiocom is the clear leader in hospital based CHF telemonitoring and telemanagement tools. The Cardiocom System is used by many of the top 100 Cardiovascular Hospital groups because it is the most comprehensive, yet cost effective solution available today.
Cardiocom's OMNIVISOR® patient telemanagement hardware and software system and COMMANDER, TELESCALE® and CARESTAR® remote patient monitoring device provide your in-house disease management team with interactive tools for objective daily weight and symptom monitoring.
Cardiocom TELESCALE® is especially effective for CHF telemanagement and has proven to:
• Improve CHF quality of care
• Reduce CHF DRG losses
• Free-up ICU and CCU beds for higher DRG revenue opportunities
• Optimize nursing resources
• Reduce prepaid MCO admissions
In a retrospective review of 1,000 NYHA Class III-IV CHF patients over a 12-month period at nine different organizations using the Cardiocom System, the heart failure admission rate was reduced to 0.234 per patient per month. This low admission rate using your own nursing staff can translate into savings for your hospital.
Hospitals nationwide have seen significant reductions in unreimbursed medical costs with the use of the Cardiocom System. Even in small CHF patient populations, dramatic savings have been shown after using the System. For example, in the year before using the Cardiocom System at St. John's Hospital, 34 of the Hospital's CHF patients incurred a total of $883,592 in medical bills. In the year since using the System, the patients' medical bills dropped to $355,390, a decrease of more than 50% which helped reduce variable, unreimbursed expenses.
In a larger hospital, data was analyzed from 71 patients in a 6-month pre and post Cardiocom period where each patient was used as their own control. In this study, inpatient admissions for CHF were reduced by 71% (admits 73 vs. 21) and inpatient costs for these CHF admissions were reduced by 73% ($315,351 vs. $83,999).
Cardiocom's COMMANDER and CARESTAR® can be used for telemonitoring multiple disease states such as COPD, Asthma, Diabetes and CAD.
The Cardiocom System is a proactive method for remote patient tracking and support that provides daily communication between providers and patients. Staying in contact with patients at risk lowers health care costs by reducing the need for lengthy, unreimbursed hospital stays.
