  • ENROLLMENT OUTREACH

    A critical component of any successful telehealth program is the ability to clearly communicate the benefit ts to your patients and enroll them in your program. We accomplish this important job by relying on our decade of telehealth experience, proven enrollment processes, and our expert enrollment professionals.

  • CLINICAL MANAGEMENT

    Our Integrated Nurse Services team intervenes at the “teachable moment” to effectively communicate patient physiological, behavioral and adherence information to managing healthcare providers. We provide essential vital sign and symptom data to the patient’s physician that is used to make important care plan adjustments. We work as an extension of your team to communicate the recommended plan of care and ensure that interventions occur in a timely manner.

  • SET-UP & ACTIVATION

    Cardiocom’s telehealth monitoring system is the most intuitive and easiest-to-use in the world. Occasionally, however, patients require basic assistance, and our Enrollment and Activation Team is skilled at helping them set-up and use their home monitoring systems.

  • TRUSTING RELATIONSHIPS

    We follow a proven process to build strong primary nurse-to-patient relationships. Each Cardiocom Telehealth Nurse Specialist cares for a specific patient and that assignment remains consistent for their duration in the program. We take the time to learn about the whole patient and collaborate with you to provide guidance and health education that is personalized for each individual.

  • PROGRAM SATISFACTION SURVEYS

    To help determine care program effectiveness, along with physician and patient satisfaction, Cardiocom routinely administers surveys including KCQOL, SF-12 and NYHC. Cardiocom has the capacity to administer surveys, provide raw data results and provide outcomes at any frequency you desire.

  • ADHERENCE

    Cardiocom’s nurses are highly experienced in coaching patients on the importance of daily monitoring, and are equally skilled at encouraging daily use and care program compliance. Through adherence outreach calls we’re able to identify patients who might have additional health concerns and address them immediately so they remain engaged and active in your program.
