To Deploy Integrated Telehealth Solution

TelehealthSM, announced a partnership to integrate the Cardiocom Telehealth
System directly into Homecare Homebase. This partnership will enable
Homecare Homebase customers to not only remotely monitor patient vitals, but
also integrate their health information automatically into their electronic medical
record.
The Cardiocom Telehealth System enables the agency to monitor patients’ daily
vital signs and symptoms. Using the patient’s phone or wireless connection, data
is transmitted in real-time from the patient’s home to the Homecare Homebase
solution. The Cardiocom Telemonitoring System guides the patient through an
interactive Health Check using a series of intuitive health and wellness-related
questions. The system collects various vital signs including: weight, blood
pressure, oxygen saturation and heart rate. The result is improved efficiency,
more proactive patient care and ultimately a better outcome for the patient.
