Golden Age Home Health Implements Telehealth Monitoring Program To Monitor Patients In The Home
OKLAHOMA CITY, OK - PR Newswire
Golden Age Home Health, a provider of home care services in Oklahoma City and surrounding areas, recently entered into a business relationship with Cardiocom to provide advanced home telehealth monitoring to patients with chronic illness. Cardiocom is a world-class developer, manufacturer and clinical service provider of award winning telehealth solutions.
Cardiocom's telehealth system provides an interactive opportunity to proactively manage patient care. Patients use the Cardiocom Commander Telehealth System to communicate with a Golden Age Home Health nurse. The Commander Home Telehealth System guides the patient through an interactive "health check" that includes the collection of symptoms, care plan compliance and vital signs such as weight, blood pressure, oxygen saturation, heart rate and glucose monitoring. This information is immediately transmitted over a standard phone line or a cellular signal to Golden Age Home Health. Physicians also have the capability to monitor their patient's reports.
"As healthcare providers, we are continuously seeking out more efficient and effective ways to deliver care to our patients," said Lydia Sowah, administrator of Golden Age Home Health. "We believe that by using the Cardiocom telemonitoring system, we can prevent more of our patients from readmissions into the hospital. With Cardiocom, we will be able to catch early signs and symptoms of exacerbation of disease process."
The Cardiocom telemonitoring system does not replace the nurse from physically going into the patient's home for their plan of care but is a value-added benefit of being able to monitor the patient when a nurse is not present which allows Golden Age Home Health to improve the quality of care for the patients by transmitting objective, real-time health data every day, the patient is connected to a nurse so that their plan of care can be adjusted as needed.
"We are delighted to be able to provide this service to our patients because along with the telemonitoring system, we are providing our patients with a peace of mind and a sense of security when they feel most vulnerable," said Sowah.
